PLEASE READ Statement of Occurence (Grievance Form) can now be filled out online and submitted via email from a mobile device, or desktop computer. For Apple Devices you would need to download the Adobe Acrobat app. Next you click on the statement of occurrence link below to get started. Once you have the form open, at the bottom of your browser click on the up arrow, then chooseImport to Acrobat, next choose Fill and Sign. In order to submit the form all fields must be completed. It is your responsibility to contact the Local at 214-826-6215 or your Chief Steward to ensure it was received
